Forms & Checklists


These permit checklists and forms are available to assist customers in submitting complete permit applications, plans, and documents. This simplifies and expedites the permit approval process. While the checklists are thorough, they are not exhaustive and do not apply to every project type.

CONTRACTORS:  Please note that your application for permits must be made through iMS, with plan submissions completed through ePlan, as applicable.


Checklists for Permit Submissions

  • Additions - Residential

    RESIDENTIAL ADDITION PERMITS

     

    Residential Addition permits are needed for adding additional space to a home. Residential Addition permits also include screen rooms, pool cages/enclosures, attached carports, and glass rooms.

     

    Residential Addition permits should be applied for online using the iMS permitting system. For further information on using the iMS system, please click here.

     

    Following submission of your Residential Addition permit application in iMS, you will receive an email notification to upload the documents listed below into our plan review system, ePlan ProjectDox. All documents must be uploaded in .pdf format.

     

    Complete Set of Sealed Plans

    A complete and concise illustration of the proposed work prepared with sufficient clarity to indicate the location, nature, and extent of the work proposed and show in detail that it will conform to the provisions of the building code, relevant laws, ordinances, rules, and regulations. A complete and detailed scope of work is required. All plan documents must be signed and sealed by the Architect or Engineer.

    All documents must be uploaded in .pdf format. Plan sheets must be uploaded as one page per file.

    Site Plan

    The Site Plan Guidelines must be followed. It must include the property lines, the footprints of existing structures, drawn and labeled streets, and proposed setbacks from the property lines.

    Florida Energy Code forms

    (If adding living space)

    Form 402-2023. This form is usually prepared, signed and dated by the Architect/Engineer of record or the A/C Contractor. These Building Code requirements range from prescribing minimum equipment ratings, materials, construction practices, etc., and are specifically governed by the 8th Edition 2023 Florida Energy Code. These forms are intended to demonstrate compliance with the energy efficiency criteria and the Building Department highly recommends that you seek professional assistance through the services of a qualified Architect, Engineer or HVAC Contractor in completing these forms when they are specifically required for your particular construction project.

    Florida Product Approval Numbers

    The Florida Product Approval Numbers are needed for exterior construction materials (roofing, windows, doors, etc.) The Florida Product Approval Numbers and installation sheets must be submitted for review. To obtain Florida Product Approval Numbers and the corresponding installation sheets, please visit www.floridabuilding.org.

    Health Department Septic Tank Permit

    If your property is served by septic system and additional living space is being constructed, a copy of the Health Department approved septic tank permit

    with the original approved stamped plans must be submitted. Please contact the Environmental Division of the Health Department at 863-519-8330 to request a septic tank permit application.

    If your property is served by a public or private sewer (non-septic) system, no such Health Department approval is required.

    Owner-Builder Statement/Affidavit

    (Applicable to Qualified Owner-Builders Only)

    An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are

    required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder. The form can be requested by emailing buildinginspection@lakelandgov.net.

    Notice of Commencement (NOC)

    The State of Florida requires a Notice of Commencement form to be submitted for all projects with a job value over $5000. The completed NOC should be taken to the Polk County Clerk of Courts to be recorded. Once recorded, the NOC must be submitted to the Building Inspection Division prior to your first inspection.

     

  • Demolition

    DEMOLITION PERMITS

     

    Before demolition can begin at a property, the sewer must be capped by a plumbing contractor. Therefore, a Plumbing sewer cap permit must first be applied for, issued, inspected, and Finaled before a Demolition permit application can be submitted.

     

    *If there is a septic tank instead of a city sewer connection, an abandonment letter from the Polk County Health Department must be uploaded with the Demolition permit application. The Polk County Health Department can be reached at 863-519-8330.

     

    Both Plumbing sewer cap permits and Demolition permits should be applied for online using the iMS permitting system. For further information on using the iMS system, please click iMS in the menu at the left.

     

    At the time of submission of your Demolition permit application in iMS, you will need to upload the following documents into the system:

     

    1. A notarized, signed letter from the property owner authorizing the demolition.
    2. If the job valuation/contract amount is over $5000, a recorded Notice of Commencement is required.
    3. A copy of the septic tank abandonment letter from the Polk County Health Dept. -OR- If the septic tank is going to be re-used for new construction, and not demolished, a pump and inspection report must be submitted. The pump and inspection report is done by a septic contractor and approved by the Polk County Health Department. The report verifies the size and condition of the septic tank and drain field and is used to determine if the septic system is compatible with the new construction.
    4. If it is a City-ordered demolition, a copy of the purchase order from the City of Lakeland must be provided.

     

    INSPECTION REQUIREMENTS

     

    ASBESTOS REMOVAL (if required) – done by an Asbestos Abatement Contractor only – The Building Inspection Division should be notified that the structures are scheduled to be demolished.

    PRE-INSPECTION OF THE SITE – Demolition contractor must schedule this inspection 24 house in advance of demolition. The Public Works, Engineering Division, will conduct the inspection consisting of existing conditions such as soil erosion, driveway aprons, sidewalks, and other items located within the public rights-of-way.

    FINAL INSPECTION BY BUILDING DIVISION – To be scheduled once the building has been demolished and removed, and the site is leveled, graded, and all debris is removed.

    FINAL INSPECTION OF THE SITE BY PUBLIC WORKS – Inspection consists of the condition of the site to determine any new damages attributed to the demolition and clean up. Appropriate erosion control measures need to be in place wherein the site shall be maintained either by vegetation and/or in a dust-free condition. Driveways shall be barricaded appropriately to discourage improper use of the site.

     

     

  • Dock

    DOCK PERMITS

     

    A Dock is a structure extending alongshore or out from the shore into a body of water, suitable for walking and/or to which boats may be moored.

     

    Before applying for a permit, please contact our Public Works Engineer at 863-834-3307 to discuss the viability of a new dock or dock work on your property.

     

    Dock permits should be applied for online using the iMS permitting system. For further information on using the iMS system, please click iMS in the menu at the left.

     

    Following submission of your Dock permit application in iMS, you will receive an email notification to upload the documents listed below into our plan review system, ePlan ProjectDox. All documents must be uploaded in .pdf format.

     

    Complete Set of Sealed Plans

    A complete and concise illustration of the proposed work prepared with sufficient clarity to indicate the location, nature, and extent of the work proposed and show in detail that it will conform to the provisions of the building code, relevant laws, ordinances, rules, and regulations. Indicate post and beam spacing and sizes, decking material, and connector details on the plans. Indicate through elevations how high the dock is above the waterline. A complete and detailed scope of work is required. All plan documents must be signed and sealed by the Architect or Engineer.

    All documents must be uploaded in .pdf format. Plan sheets must be uploaded as one page per file.

    Site Plan

    The site plan must include the property lines, the footprints of existing structures, drawn and labeled streets, proposed dock, location of turbidity barriers, and setbacks from the property lines.

    Owner-Builder Statement/Affidavit

    (Applicable to Qualified Owner-Builders Only)

    An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder. The form can be requested by sending an email to buildinginspection@lakelandgov.net.

  • Retaining Wall/Masonry Wall

    FENCE / RETAINING WALL PERMITS

     

    The City of Lakeland is currently not requiring or accepting applications for fence permits. While fence permits are not required to install a new fence or replace existing fencing, we strongly encourage property owners to contact the Planner on Demand at (863) 834-7526 or Planning@Lakelandgov.net prior to installation to ensure compliance with Land Development Code Requirements.

     

    Any new or replacement fences on properties located in a historic district must still obtain a Certificate of Review prior to installation. For information on Certificates of Review, please contact (863) 834-6094 or historicpreservation@lakelandgov.net.

     

    Fences which do not meet these requirements are subject to code enforcement action.

     

    *Permits are required for masonry walls and retaining walls.*

    Complete Set of Sealed Plans

    Please provide a complete and concise illustration of the proposed work prepared with sufficient clarity to indicate the location, nature and extent of the work proposed and show in detail that it will conform to the provisions of the code, relevant laws, ordinances, rules and regulations. All required pages must be signed and sealed by an Architect or Engineer. A complete scope of work is also required to be submitted describing the work being done. All documents must be uploaded in .pdf format. Plan sheets must be uploaded as one page per file.

    Site Plan

    The site plan must include the property lines, the footprints of existing structures, drawn and labeled streets, proposed wall location(s), proposed setbacks from the property lines, height of the proposed fence, and materials used for the proposed fence. Block wall fences will require detailed drawings and may be required to be engineered.

    Owner-Builder Statement/Affidavit

    (Applicable to Qualified Owner-Builders Only)

    An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder. The form can be requested by sending an email to buildinginspection@lakelandgov.net.

    Notice of Commencement (NOC)

    The State of Florida requires a Notice of Commencement form to be submitted for all projects with a job value over $5000. The completed NOC should be taken to the Polk County Clerk of Courts to be recorded.   • Generator

    GENERATOR PERMITS

     

    Generator permits should be applied for online using the iMS permitting system. For further information on using the iMS system, please click iMS in the menu on the left side of the page.

     

    Following submission of your Generator permit application in iMS, you will receive an email notification to upload the documents listed below into our plan review system, ePlan ProjectDox. All documents must be uploaded in .pdf format.

     

    1. Complete Site Plan, with dimensions, of the property. The Site Plan must show:
    • The proposed location of the generator & all existing structures and electrical equipment.
    • The distance from the generator to the property lines.
    • The distance from the generator to adjacent structures.
    • The distance from the generator to openings such as doors, windows, and intakes.
    • The distance from all structures and lot lines to the fuel source, including the location, size, and type of fuel source (gasoline, diesel, compressed natural/propane tanks).
    • The construction type of all structures within five feet of the generator.
    1. The manufacturer’s installation instruction sheets, including foundation/slab anchoring requirements.
    2. A one-line electrical diagram.
    3. A complete load calculation per NEC Art. 220 when automatic transfer switches (ATSs) are used. Generator must be sized for calculated load. Provide a list of managed loads and gas equipment.
    4. A fault study with short circuit amps (SCA) at new electrical equipment. Fault study can be omitted if utility transformer SCA is less than the short circuit current rating (SCCR) of ATS or MTS. Provide Lakeland Electric transformer SCA. The SCCR of the ATS must be greater than the transformer SCA or a fault study will be required.
    5. Generator and ATS specifications. This information will be used to determine the generator and ATS are properly sized and the SCCR of ATS is adequate for calculated SCA in fault study.
    6. Provide specifications for load management devices.
    7. Provide a note on plans requiring signs at service entrance equipment per 702.7 (A) and 702.7 (B).
    8. Owner-Builder Statement/Affidavit (Applicable to Qualified Owner-Builders Only). An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder. The form can be obtaining by emailing buildinginspection@lakelandgov.net.
    9. Notice of Commencement (NOC). The State of Florida requires a Notice of Commencement form to be submitted for all projects with a job value over $5000. The completed NOC should be taken to the Polk County Clerk of Courts to be recorded. Once recorded, the NOC must be submitted to the Building Inspection Division prior to your first inspection.
  • Mobile Home Setup

    MOBILE / MANUFACTURED HOME SETUP PERMITS

     

    Mobile/Manufactured Home permits are needed for the set-up of a new mobile or manufactured home within a mobile home community. This includes replacement of an existing or previous mobile/manufactured for a new one on the same lot or property.

     

    *If the manufactured home will be placed on a lot/parcel where single family homes are permitted, you must apply for a Building Residential / New Single Family permit.

    Mobile Home Permit Worksheet

    Include the City of Lakeland Permit Worksheet along with the permit package. Along with this worksheet, please include the Soil Test and the Manufacturer’s information on the brace.

  • Remodel- Residential

    RESIDENTIAL REMODEL / ALTERATION PERMITS

     

    Residential Remodel permits are needed for making changes to a home, including but not limited to reconfiguration of the space or rooms, elimination of a door or window, siding, removal or installation of walls, enclosing carports, structural changes along with all corresponding trade work (electrical, mechanical, plumbing, roofing). This list is not exhaustive. If you have questions about your specific project, please call the Building Division at 863-834-6012.

    Site Plan

    If changes are being made to the exterior of the home such as the replacement or installation of siding or stucco, or the elimination of a door or window, a site plan is needed. A drawing of the outline of the structure and indicate where the siding is to be installed or the door/window is to be eliminated. This assists the inspector when identifying the area of construction. If the siding is around the entire structure, no site plan is required.

  • Roofiing

    ROOFING PERMITS

     

    Roofing permits are needed for re-roofing residential and commercial properties, along with additional structures such as accessory dwelling units, detached garages, sheds, storage buildings, etc. Roof repairs also require roof permits. Roofing permits are needed for shingle, metal, tile/slate, modified bitumen, TPO, and composite roofs.

     

    Permits should be applied for online using the iMS permitting system. For further information on using the iMS system, please click iMS in the menu on the left side of the page. If your roofing project requires plan review, following submission of your permit application in iMS, you will receive an email notification to upload the documents listed below into our plan review system, ePlan ProjectDox. All documents must be uploaded in .pdf format.

     

    Shingle, TPO, Modified Bitumen Roofs:

    1. Florida Product Approval Numbers for roofing material and underlayment being used must be included in your application. The Florida Product Approval numbers are set by the State of Florida as a measurement to determine if the product meets state building code requirements including high velocity hurricane zones, HVHZ. The Florida Product Approval Number can be obtained from the point of purchase of your roofing materials or via the DBPR (Department of Business & Professional Regulation) at www.floridabuilding.org.

     

    2. A Letter of Structural Integrity for the roof is required. This letter should be written by the contractor or a structural engineer (or homeowner if applying for Owner Builder permit) declaring that the structural roof components are capable of supporting the roof-covering system and the material and equipment loads that will be encountered during installation of the system.

    Metal, Tile/Slate, Composite Roofs:

    1. The Florida Product Approval Numbers are needed for the roofing and underlayment materials. The Florida Product Approval Numbers and installation sheets from the DBPR (Department of Business and Professional Regulation) must be submitted for review. To obtain Florida Product Approval Numbers and the corresponding installation sheets, please visit www.floridabuilding.org.

     

    2. A Letter of Structural Integrity for the roof is required. This letter should be written by the contractor or a structural engineer declaring that the structural roof components are capable of supporting the roof-covering system and the material and equipment loads that will be encountered during installation of the system.

    If a metal roof is to be installed over existing shingles, the metal manufacturer must have approved that method of installation without an underlayment, the letter must also state that the shingles are in good condition and will provide sufficient water barrier.

    Owner-Builder Statement/Affidavit

    (For Qualified Owner-Builders Only)

    An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder.   • Seawall

    SEAWALL PERMITS

     

    A seawall is a structure that provides a defense to the shoreline and surrounding property from erosion and flooding.

     

    Before applying for a permit, please contact our Public Works Engineer at 863-834-3307 to discuss the viability of a new seawall or seawall work on your property.

  • Shed/Garage- Sit Built

    SITE BUILT SHED / GARAGE PERMITS

     

     A site-built shed or garage is one that is constructed on site rather than being delivered in one piece and anchored down. Site-built sheds or garages are considered Accessory Structures.   • Shed- Premanufactured

    PRE-MANUFACTURED SHED PERMITS

     

    Sheds are considered Accessory Structures and require a permit. However, pre-manufactured Sheds less than 250 cubic feet AND no more than 5 feet tall from the soil to the peak of the roof do not require a permit.     Following submission of your permit application in iMS, you will receive an email notification to upload the documents listed below into our plan review system, ePlan ProjectDox. All documents must be uploaded in .pdf format.

     

    Complete Set of Manufacturer’s Plans

    A copy of the manufacturer’s plan drawings for the shed assembly. These are engineered drawings from the manufacturer explaining the limitations and installation of their product. Plans must include the height of the shed and they must include the anchoring system used.

    All documents must be uploaded in .pdf format.

    Site Plan

    The Site Plan must include the property lines, all existing structures, drawn and labeled streets, and proposed setbacks from the property lines to all sides of the proposed structure.

    Health Department Septic Tank Permit

    If your property is served by septic system and additional living space is being constructed, a copy of the Health Department approved septic tank permit with the original approved stamped plans must be submitted. Please contact the Environmental Division of the Health Department at 863-519-8330 to request a septic tank permit application.

    If your property is served by a public or private sewer (non-septic) system, no such Health Department approval is required.

     

  • Tents

    TENT PERMITS

     

    • Tents that are less than 120 square feet do not require a permit.
    • Tents 121 square feet and greater require a permit that is reviewed by the Planning & Zoning department.
    • Tents 900 square feet and greater, including revival tents, will be reviewed by the Planning & Zoning department as well as the Fire department.

     

    Once issued, a tent permit is valid for up to fifteen (15) days only.

    Site Plan

    The Site Plan must include the following:

    ·        Street address.

    ·        Proposed tent location on the property.

    ·        Distance from tent to property lines and buildings.

    ·        If multiple tent sizes will be used, the different tents should be identified on the site plan.

    Certificate of Flame Resistance

    Provide a copy of the Certificate of Flame Resistance showing that the materials used in the manufacture of the tent are flame resistant.

    Letter of Authorization

    Letter from the business/property owner granting approval to erect the tent at the location if the tent is 900 square feet or greater. The letter must include the date, the address of the property, the owner’s name and signature, the owner’s phone number and email address.

     

  • Window/Door/Hurricane Shutters Replacement

    WINDOW / DOOR PERMITS

     

    If you are replacing a window, exterior door, or garage door a permit is needed. A Window/Door permit is for the replacement of

    size-for-size windows and doors only. If you will be increasing or decreasing the size of the window or door, changing a window into

    an exterior door or sliding glass door, or closing in or eliminating a window or door, or installing hurricane shutters, a Residential Remodel/Alteration permit would be needed instead.

    The Florida Product Approval Numbers are needed for exterior construction materials (roofing, windows, doors, etc). The Florida Product Approval Numbers and installation sheets must be submitted for review. To obtain Florida Product Approval Numbers and the corresponding installation sheets, please visit www.floridabuilding.org.

    Site Plan

    A drawing of the outline of the structure and indicate which windows and/or doors are being replaced. This assists the inspector when identifying the area of construction.

    Owner-Builder Statement/Affidavit

    (For Qualified Owner-Builders Only)

    An Owner/Builder Disclosure Statement is a legally binding oath required by Florida Statute, which you are required to execute in order to be issued a building permit in your name. The Disclosure Statement affirms your certification of compliance with the limitations imposed upon you by Florida Statute 489 in order to be exempt from professional licensure requirements as an Owner/Builder. A deck might also require a railing for safety, depending on the specifics of the project.

    Florida Energy Code forms

    (If Applicable)

    Form 402-2023. This form is usually prepared, signed and dated by the Architect/Engineer of record or the A/C Contractor. These Building Code requirements range from prescribing minimum equipment ratings, materials, construction practices, etc., and are specifically governed by the Florida Energy Code. These forms are intended to demonstrate compliance with the energy efficiency criteria and the Building Department highly recommends that you seek professional assistance through the services of a qualified Architect, Engineer or HVAC Contractor in completing these forms when they are specifically required for your particular construction project.

    These are guidelines/manuals from the manufacturer explaining the proper installation of their products. This would be needed for the pool and the pump.

    Site Plan– FOR IN-GROUND AND ABOVE-GROUND HOT TUBS/SPAS

    The Site Plan must include the property lines, the footprints of existing

    structures and proposed setbacks from the property lines. The Site Plan must also show how and where the pool pump will be connected to electricity.

    Owner-Builder Statement/Affidavit – FOR IN-GROUND AND ABOVE-GROUND HOT TUBS/SPAS.

    (Applicable to Qualified Owner-Builders Only)

    setbacks from the property lines. The Site Plan for a pool must also show the fence and location of any gates and pool safety barrier, location of any doors and windows that fall within the pool safety barrier, and how and where the pool pump will be connected to

    electricity.

    Health Department Septic Tank Permit - FOR INGROUND AND ABOVE-GROUND POOLS (if applicable)

    If your property is served by septic system and additional living space is being constructed, a copy of the Health Department approved septic tank permit with the original approved stamped plans must be submitted. A complete and detailed scope of work is required, along with a structural plan, roof plan, electrical calculations, one-line electrical diagram, array and module plan, and overcurrent protection plan, along with any other documents pertinent to the project. All plan documents must be signed and sealed by the Architect or Engineer. Plan sheets must be uploaded as one page per file.
